The hospice at home service provides hands on nursing care to people thought to be close to the end of their lives, helping to support them to be at home if this is their preferred place of care, or to support them pending a hospice bed being available.
The aim of this service is to bring the benefits of hospice care to people in their own homes.
The team will assess and support the needs of the patient, working with the community nursing team, the patient’s GP, Macmillan Nurses and the Consultants. The service works between 10pm and 8am, when no other visiting nursing services are available.
They Hospice at Home team provide the peace of mind that a trained nurse can attend at night, should this be required. The service can at times provide a night sitting service.
